Thrombocytopenia Diagnosis and Treatment by Hematologist

What Is Thrombocytopenia?

Thrombocytopenia is a medical condition characterized by an abnormally low number of platelets (thrombocytes) in the blood. Platelets are essential for normal blood clotting, forming plugs at sites of blood vessel injury to stop bleeding.

A healthy platelet count typically ranges between 150,000 and 450,000 platelets per microliter of blood. When the count falls below this range, thrombocytopenia develops, increasing the risk of bruising and bleeding.

Symptoms of Thrombocytopenia

Symptoms can vary from mild to severe depending on how low the platelet count becomes. Some people may have no noticeable symptoms, while others experience significant bleeding.

  • Easy or excessive bruising (purpura)
  • Pinpoint reddish-purple skin spots called petechiae, often on the lower legs
  • Prolonged bleeding from cuts
  • Spontaneous bleeding from gums or nose
  • Blood in urine or stools
  • Heavy menstrual bleeding
  • Fatigue
  • Enlarged spleen (splenomegaly)

Causes of Thrombocytopenia

Thrombocytopenia may result from reduced platelet production, increased destruction, or abnormal trapping of platelets in the spleen.

Decreased Platelet Production
  • Bone marrow disorders: Leukemia, aplastic anemia, myelodysplastic syndromes
  • Nutritional deficiencies: Vitamin B12 or folic acid
  • Viral infections: Hepatitis C, HIV
  • Medications: Chemotherapy and certain drugs
Increased Platelet Destruction
  • Immune thrombocytopenia (ITP)
  • Drug-related immune reactions: Heparin-induced thrombocytopenia
  • Infections
  • Serious conditions: TTP or DIC
Sequestration in the Spleen
  • Enlarged spleen: Cirrhosis, lymphoma, or related diseases

Treatment for Thrombocytopenia

Treatment depends on the cause and severity of thrombocytopenia. Mild cases may only require observation, while severe cases demand urgent medical care.

  • Mild cases: Monitoring without immediate treatment
  • Medications: Corticosteroids, IV immunoglobulin, or drugs that boost platelet production
  • Platelet transfusions: For severe bleeding or extremely low counts
  • Treating the cause: Stopping an offending drug or treating infection
  • Splenectomy: Surgical removal of the spleen in selected cases
